Adds a working-agreement section directing agents to surface genuine user
decisions as button questions (AskUserQuestion) with a one-line recap of the
choice, rather than burying a yes/no in prose.

Does not change whether to ask — obvious defaults are still taken silently.
It's about how the legitimate decisions (including the confirmations the safety
rules already require for irreversible actions) are presented.

Docs-only: single new section in the top-level CLAUDE.md. No draft per the
pure-docs PR convention.
